Problems regulating T Levels

I'm having problems getting in to an optimal testosterone range.

I have had bouts of extreme fatigue the last three years and after finally convincing my doctor to prescribe testosterone, I am still not better. The dosage is 200mg once per week.

I have been on this dosage for two months and my T levels have gone from 217 to 300 to 390 ng/dl.

Should I be at 700ng/dl as I am 35 years old? I have had a lot of blood work and my adrenals are also weak so I have been on 30 mg of Cortef per day as well.

The fatigue is stopping me from enjoying my two young boys. The doc keeps saying give it at least six months to get everything regulated. Please help with any suggestions!

Hi Scott.

At 200mg a week your levels should be much higher than you are currently showing.

I'm guessing your estrogen may be a little on the high side which often times makes men feel worse during therapy.

I'd suggest talking with your doctor about getting your levels checked. If it is high, going on an anti-estrogen medication called Arimidex, starting at 0.25mg every other day, usually solves the problem. The anti-estrogen Aromasin is another option.

Ideally your estradiol levels should be around 20-30 pg/ml. If they are over 50 pg/ml many men will often feel tired and bloated, or may develop an enlarged prostate and/or sexual dysfunction as well.

You may also want to ask your doctor for a thyroid panel, looking for hypothyroidism. If your adrenals and testosterone are low there's a good chance your thyroid may be as well.

After two months there's a good chance you'll be feeling better if your hormones are dialed in. And yes 700 ng/dl would be a good target to shoot for.

Dear Doctor-

My husband started feeling fatigued, we thought it was working so much, but it never passed. Doctors thought it could be depression, but after three years we figured out it was low testosterone, his level was 200! Scary! He has been taking cypionate injections for 18 months. His levels are still under 400. He is currently taking 250mg every ten days. Something is wrong, and we need help. He is moody, upset over little things, and tells me something isn't right he needs help and is still getting exhausted 7-8 days into the IM injection. He takes thyroid meds for an underactive thyroid. We don't even know what questions to ask regarding this testosterone thing. Any suggestions?
